City MP's Mark Lancaster and Iain Stewart have visited Age UK to hear about their Take Home and Settle scheme.
Age UK have been working with MK Hospital to try and reduce the number of elderly people who are avoidably being admitted to hospital. The scheme has seen employees of Age UK based at the hospital in order help patients in the A and E and wards ensure they have the correct support to be sent home as quickly as possible.
Similar schemes are currently in operation in East Sussex and Blackpool and have seen hospitals make significant savings as elderly patients spend less time in hospital.
Mark said; 'I am really impressed at how Age UK and the hospital are working together to support elderly residents and stopping unnecessary hospital admissions. This kind of integrated health and social care service is to be commended and shows once again how well organisations in Milton Keynes work in partnership.'
Iain commented; "This is a really useful initiative that prevents vulnerable elderly residents from needlessly staying overnight in the hospital. Take Home and Settle is just one of the many initiatives to improve the social care experience for residents and I commend Age UK for their work."
